Team 265 is an initiative between the Play For Jake Foundation and the Zac Mago Foundation to honor both Jake West and Zac Mago who died from Sudden Cardiac Arrest.
While we remain separate foundations, we have joined forces to provide Preventative Heart Screenings to all those aged 10 to 25, CPR Training and AED Placement.

Our amazing initiative is led by both Jake's mom, Julie, and Zac's mom, Teresa. They are determined to do all they can to bring awareness, education and prevention to conditions that could lead to Sudden Cardiac Arrest. The organization is also supported by volunteers and the people who know the importance of prevention of SCA.

Our Story
Jake West and Zac Mago both lost their lives to Sudden Cardiac Arrest. No signs, no symptoms, no warnings, just gone. Both boys passed every pre-participation sports physical and well check. In their honor, their loved ones started foundations to prevent SCA in other young people. In early 2021, The Zac Mago Foundation and Play For Jake Foundation came together to become Team 265.
